- You must have a valid e-mail address. This is how we will communicate with you.
- All fields must be completed.
- An appointment is required to enter the passport agency. We are unable to accommodate walk-ins. You will not be allowed into the building if you do not have an appointment and your required documentation.

Make an appointment here to apply at a passport agency, if:
- You may schedule an appointment if you are traveling within 14 days or need a foreign visa within 4 weeks.
- You will be able to provide proof of travel (such an airline ticket receipt or itinerary) at the time of your appointment. If you are traveling by land or sea to Canada, Mexico, Bermuda, or the Caribbean, you can provide a hotel reservation, cruise tickets, or international car insurance.
- You agree to pay the additional $60 expedite fee
- You do not already have an appointment; duplicate appointments may result in all of your appointments being cancelled.
If you have a disability and would like to request an accommodation due to the disability when you apply at a passport agency:
- Make the appointment online
- Make note of your appointment confirmation number
- Call 1-877-487-2778 [1-888-874-7793 for TDD/TTY teletype service] and reference your confirmation number when asking for the accommodation request to be added to your appointment
